Step 2: We Gather Documentation
Patient assistance applications require documentation — typically proof of income, proof of insurance status (or lack thereof), and a signed prescription.

- A recent tax return or pay stubs (or a signed declaration if income is difficult to document)
- Your prescription information (we work with your pharmacy and doctor's office directly)

Step 3: We Coordinate with Your Doctor's Office
This is where most self-navigated applications get stuck. BMS Access Support requires a provider attestation — your doctor or their office needs to sign off on the application and confirm the medical necessity of Eliquis.
