How Botox Therapeutic Works

Botulinum toxin type A blocks the release of acetylcholine — the neurotransmitter that signals muscles to contract and glands to secrete. At the neuromuscular junction, Botox binds to the nerve terminal and prevents synaptic vesicles from fusing with the membrane, stopping acetylcholine release. The result is a localized, reversible chemical denervation: targeted muscles relax or become temporarily paralyzed, and overactive secretions are reduced. In chronic migraine, the mechanism is thought to involve blocking the release of peripheral pain-signaling neuropeptides (like CGRP and substance P) from sensory neurons. Effects last approximately 3–4 months before new nerve terminals form and function is restored.
